Liability Waiver, Medical Authorization, Media Release, and Parent Duty Waiver Deposit Agreement
责任豁免、医疗授权、肖像使用及家长值班豁免押金协议
1. Definitions 定义
Indianapolis Chinese Community Center Inc. Chinese School（“ICCCI Chinese School”或“学校”）包括其所有管理人员、董事、员工、教师、志愿者、承包方及授权代表。
2. Assumption of Risk 风险承担
I understand that participation in School activities involves inherent risks, including but not limited to physical injury, illness, property damage, or other harm. I voluntarily assume all such risks for myself and/or my child, including transportation to and from activities.
本人理解参与学校活动存在固有风险，包括但不限于人身伤害、疾病、财产损失或其他风险。本人自愿承担本人及/或未成年子女参与相关活动（包括往返交通）的一切风险。
3. Release of Liability 责任豁免
To the fullest extent permitted by Indiana law, I release, discharge, and hold harmless the School from any and all claims, liabilities, damages, or injuries arising out of participation in School activities, including those resulting from the ordinary negligence of the School.
在印第安纳州法律允许的最大范围内，本人同意免除学校因参与活动所产生的一切责任，包括因学校一般过失导致的索赔、损害或伤害。
4. Indemnification 赔偿责任
I agree to indemnify and hold harmless the School from any claims, damages, or expenses (including reasonable attorney’s fees) arising out of participation, except to the extent caused by the School’s gross negligence or willful misconduct.
本人同意对因参与活动产生的任何索赔、损失或费用（包括合理律师费）对学校进行赔偿并使其免责，但因学校重大过失或故意不当行为导致的除外。
5. Medical Authorization 医疗授权
In the event of an emergency, I authorize the School to obtain or arrange medical treatment for myself or my child. If I cannot be reached, the School may act in its reasonable discretion. I agree to assume full financial responsibility for any resulting costs.
在紧急情况下，本人授权学校为本人或未成年子女安排或获取医疗救助。如无法联系本人，学校可基于合理判断采取行动。相关费用由本人承担。
6. Photo / Video Release 肖像与影像授权
I grant the School a worldwide, perpetual, royalty-free, non-exclusive right to photograph, record, and use my or my child’s image, likeness, voice, and recordings for educational, promotional, and publicity purposes, including but not limited to websites, social media, publications, and displays.
I waive any right to inspect, approve, or receive compensation arising from such use. To the fullest extent permitted by law, the School shall not be liable for unauthorized use by third parties.
本人授权学校在全球范围内永久、无偿、非独占性使用本人或未成年子女的肖像、影像、声音及相关记录，用于教学、宣传及推广（包括但不限于网站、社交媒体、出版物及展示等）。
本人放弃对上述使用的审查权及获取报酬的权利。在法律允许范围内，学校不对第三方未经授权的使用承担责任。
7. Parent Duty Waiver Deposit 家长值班豁免押金条款
The School encourages parent participation in volunteer duties (“Parent Duty”) to support school operations.
A $50 deposit is required for families who choose to waive Parent Duty obligations.
This deposit serves as a Parent Duty waiver fee and is non-refundable.
Families who fulfill assigned Parent Duty responsibilities may not be required to pay this deposit, or may receive a refund if previously paid (subject to School policy).
Failure to fulfill agreed Parent Duty obligations, if selected, will result in forfeiture of the deposit.
学校鼓励家长参与志愿值班（“家长值班”）以支持学校运营。
选择不参与家长值班的家庭需支付 $50押金
该押金作为家长值班豁免费用，不予退还
如家庭履行家长值班义务，则无需支付押金或可根据学校政策退还已缴押金
如已选择参与但未履行值班义务，押金将被没收
